The Communicable Disease Prevention Unit sends out periodic immunization updates known as the Vax Fax to San Francisco health care providers.  Health care providers that would like to receive Vax Fax and Health Alerts please fill out our online Health Alert Notification Database (HAND) Request Form.  All contact information is confidential.
